Hazard: Due to a programming defect with the console, the treadmill can unexpectedly increase in speed and elevation when the User Programs 4 or 5 are set for longer than 30 minutes. If this happens, the user could fall and suffer injuries.

Hamilton Medical Issues a Medical Device Safety Alert on certain RAPHAEL Ventilators
Hamilton Medical, Inc. announced today that it is initiating a nationwide voluntary recall of forty seven (47) RAPHAEL ventilators with older generation software. Current RAPHAEL Color and RAPHAEL XTC ventilators are not affected by this action. Only four (4) customer facilities have been impacted by this action. Hamilton Medical, Inc. has already contacted these facilities regarding immediate field corrections.
This voluntary recall is being conducted because the RAPHAEL ventilator, under certain very specific conditions following an oxygen cell calibration without a compressed air supply, as instructed in the RAPHAEL Operator's Manual, can be put into a state where no visible or audible alarms are triggered. Hamilton Medical investigated a single complaint and found that a software algorithm designed to suppress false positive alarms may preclude any alarm under this scenario. "It should be noted," states David Costa, Vice President for Hamilton Medical, Inc., "that the oxygen cell calibration is intended to be performed while the RAPHAEL is connected to both air and oxygen high pressure gas sources. This could cause the sliding resistance of the piston to be excessive, which in turn could cause the return of the piston to be slow upon release of the brake. The piston may not return completely. If this happens, the rear brake light could remain illuminated which could cause a crash or injuries.

Audi 1998-2000 A6, A6 Avant, S6, and S6 Avant Passenger Vehicles (Fri, 02 Jun 2006) On certain passenger vehicles, while performing the remedial repair for recall 04V133000, if the plastic cover is not securely installed to the driver's side support crossmember, the headlight switch wiring harness could come into contact with the crossmember. This could lead to an electrical short in the harness, which could lead to a fire in the driver's side dashboard area.
Federal-Mogul and Carquest Aftermarket Hub Assembly Recall
Carquest, National 515058 Hub Assemblies for 2001-2006 GMC 1500, 2500 Trucks (Thu, 01 Jun 2006) Certain Federal-Mogul National and Carquest aftermarket hub assemblies, P/N 515058, sold for use on 2001-2006 General Motors 1500 and 2500 4WD trucks. On the subject hub assemblies, the ABS sensor cord departs from the hub assembly at a 90 degree angle. As a result, there is a fit issue with the brake rotor dust shield interfering with the ABS sensor cord. If the hub assembly is installed despite this interference, the gap between the ABS sensor and the tone wheel may be altered. This may weaken the output of the ABS sensor. Diminished output could result in an unwanted ABS activation at low speeds and stopping distance could be affected. If stopping distance is limited, a vehicle crash could occur.

Firestone Destination Passenger Tires (Wed, 31 May 2006) Certain Firestone Destination A/T P265/75R16 passenger tires manufactured between January 29 and February 4, 2006. Due to an improper machine setup, the tires may have been produced with a damaged innerliner which could result in a small area on the inside of the tire to have no innerliner coverage. Tires built under such conditions may experience air permeation into the body of the tire. Over time, this may lead to tread separation and air loss, possibly resulting in a vehicle crash.

Defendants Will Pay Nearly $1 Million in Consumer Redress and Penalties
A credit counseling service and related companies and individuals have agreed to pay $926,754 in consumer redress and civil penalties to settle Federal Trade Commission charges that they made false claims about their debt management program and violated the FTC’s Do Not Call Rule.
According to the FTC, Credit Foundation of America, Inc. (CFA), and its associates sold debt management services nationwide through unsolicited, pre-recorded messages left on home telephones, falsely claiming that consumers were pre-approved for a program to consolidate their credit card debts to a single monthly payment at a much lower interest rate. When consumers responded to the calls, they were encouraged to enroll in a debt management plan, regardless of their individual circumstances. Many enrollees were not appropriate candidates for debt management plans and lost the large enrollment fees the defendants charged. Read more
